The Crisp County School District is gearing up for another successful school year and would like to share important information regarding student vaccinations and immunizations.
Per the Georgia Department of Public Health,
"Children attending any childcare facility, pre-kindergarten, Head Start program, nursery, or school in Georgia are required to have the Georgia Certificate of Immunization, Form 3231 on file for all children through 12th grade. This includes public and private operations and all enterprises, educational programs, and institutions involved in the care, supervision, or instruction of children."
To make sure your child's immunization/vaccination requirements are up to date, you can contact your local health department, your child's doctor, or your child's school.
